🕒 Please arrive 30 minutes prior to your appt for registration. No tests can be undertaken without a stamped request form

If you are having an ECG or Holter monitor -you should report to the Diagnostic OP Centre (past Mews House 6 - just opposite the main hospital building) to collect and stamp a test request form and have the ECG

· Otherwise, please go to Level 1, Paediatric Outpatients (for the Consultation & Echo which will be done after a request form has been completed

 

 

For parents with a baby or infant we recommend bringing a pacifier or a bottle-of-milk to help during the scan.

​

 

Please bring your insurance details (policy number and authorization number). If pre-authorisation is not granted prior to the consultation, you will be asked to pay by cash or card on the day of your visit prior to your consultation.

 You should then forward the receipt to your insurance company for reimbursement.
